Apprenticeship sponsors play a crucial role in providing high-quality training experiences for Maryland’s Apprentices. Every registered sponsor has access to FREE ongoing support from a Maryland Department of Labor Apprenticeship Navigator. The forms below are for sponsors of apprenticeship programs already registered with the Maryland Apprenticeship and Training Council. Please submit completed forms to the address at the bottom of this page. The Office of Apprenticeship offers several training, development, and outreach opportunities for Registered Apprenticeship sponsors.
